Introducing Our New Care Management Program With Nura Health
At Castle Rock Primary Care, we are committed to providing care that supports you not only during office visits, but also between appointments. We are pleased to share that we have joined the Nura Health care management program.Schedule Now
Care management is designed to help eligible patients with ongoing health needs stay connected to their care team, better understand their treatment plan, and receive support in managing their health outside the clinic.
Through this program, patients may receive assistance with:
- Coordinating care among members of their healthcare team
- Reviewing medications and care plans
- Setting health goals and tracking progress
- Addressing barriers that may make it harder to follow a treatment plan
- Connecting with helpful community or healthcare resources
- Receiving support between routine office visits
Participation is voluntary and may be especially helpful for patients managing multiple chronic health conditions or those who would benefit from additional guidance and follow-up.
