We provide registration paperwork ahead of time. The following forms have been provided for your convenience and to shorten you waiting time in the office. The forms may be printed and filled out prior to your appointment.

Patient Information Sheet - A new Patient Information Sheet is required annually. If you have not updated your information lately, please bring the completed form with you.

Adult Health History - If you are a new patient or have had significant changes in your health status, please complete this form and bring it with you.

Parental Pre-Authorization - If you are sending your child for an office visit without a parent present, please complete this form ahead of time.

Records Release - If you plan to move all your records to Founders Family Medical Center, fill out this form, sign and give it to your old doctor’s office. They will fax or send your records to us.

How Can We Reach You - This form is required for new patients.
